There is a book by Clair Davies entitled The Trigger Point Therapy Workbook: Your Self-Treatment Guide for Pain Relief. It is an amazing manual for self treatment. It helps you understand how the muscle fibers get tensed out and how you can manage the pain yourself. All you need is a tennis ball or raquetball and floor, chair, and wall. There are other instruments you can purchase to help release the trigger points, such as the knobble, but the balls really work quite well. I used to have to use them for hours every single day.
I went to get a massage in San Francisco from this woman who first told me about the book. Fortunately they sold it there and I purchased it. I used that book religiously before I found Brain Highways. (You can find it on Amazon.com.) It helped so much to deal with the chronic pain issue when I could not get the pain to go away. I also found a trigger point massage therapist who is amazing! Again he is local to the Orlando area. He works at Harris Chiropractic in Oviedo, Florida, and his name is Sean. If you live around here, I recommend him highly. He's excellent at working out trigger points, especially in the neck, back, and shoulders. My scalenes and SCM muscles used to be so tight and so very painful, and he worked them right out for me. I used to have to keep going back because the muscles do tense back up unless you can find the source of the tension.
